His hand upon my heart comforts so tenderly
Patiently He knits the ragged edges of my heart
Sin had left me broken; torn completely apart
As the seams make my heart again whole
Hope blossoms and soon takes hold
Promises from His word ring true
Restoring my soul to its unblemished hue
Praises and thanksgiving exalt from my lips
Landing upon all like a sweet treasured kiss
The darkness and peril of sin is replaced
The precious gift of forgiveness remains in its place
No needle or thread in His mending He applies
The holes in His hands and feet are the ties that bind
His life so freely given on the cross at Calvary
Marked the end of sin as death for me
His gift of salvation is offered to all
Awaiting only a sincere heart’s call
Neither shame nor defeat will I again feel
For it’s at the Lord’s throne that I kneel

So much of what makes a poem good is here: lovely word choice and imagery, rhyme, and a thoughtful message. A bit more attention to meter would really add polish to this entry.
