SUBJECTIVE:
CHIEF COMPLAINT(S): Suspected writer's block.
Chrissy is here to request assessment for suspected writer's block. She is an author and reports increasing "brain fog" and states that she "draws a blank" every time she tries to write. Symptoms began approximately four weeks ago and continue to worsen. She also experiences sweating and lightheadedness at the sight of a blank page on her computer screen.
She also states that she is a Christian and wishes for her writing to bring glory to God. This has added an increased level of distress in regards to her inability to write.
Area involved: Brain.
Onset of symptoms: One month ago.
Reports condition is: Worsening.
Duration of symptoms is: Acute.
Usually associated with: Attempting to write.
Improved by: None/nothing.
Has tried: Taking long walks, stretching, brainstorming, making lists.
REVIEW OF SYSTEMS:
GENERAL-CONSTITUTIONAL: Reports symptoms of fatigue.
Denies symptoms such as fever or weight loss.
PSYCH-PSYCHIATRIC: Reports symptoms of anxious-nervous-tense and/or apprehension.
Denies symptoms such as sleep problems.
NEURO-NEUROLOGICAL: Reports symptoms of dizziness.
Denies symptoms such as headache.
MUSCULOSKELETAL: Reports symptoms of neck pain or problems.
Denies symptoms such as joint pain or problems.
OBJECTIVE:
VITAL SIGNS:
BMI: 25.7.
Blood pressure: 120/80.
Height: 65 inches.
Pulse: 80 beats per minute.
Respiratory Rate: 24 breaths per minute. Temperature: 98.3 F.
Weight 154 pounds, 0 ounces.
EXAM:
GENERAL: General appearance can be described as well-nourished, well-developed, and in no acute distress.
MUSCULOSKELETAL: Inspection-Palpation: No bone, joint, tendon, or muscle abnormalities.
NEUROLOGICAL: Alert and oriented.
PSYCHIATRIC: Insight and judgement both appear to be intact and appropriate.
Mood and affect are normal.
ASSESSMENT: Writer's Block - Severe
PLAN:
Chrissy appears normal on exam today. However, based on her reported symptoms, I feel she is experiencing severe Writer's Block.
I spent considerable time with the patient reviewing this condition and its proper treatment. Treatment prescribed as documented below. Chrissy was informed that the remedy may be perceived as dry and distasteful when it is first experienced. She will need to persist in applying it until it becomes sweet. At that point, she can adjust its application to whatever amount she desires. Use it at least once daily for full effect. The treatment is not harmful in any way and she may continue it indefinitely.
PRESCRIPTION:
One hour of heartfelt prayer per day for seven days, then as needed.
FOLLOW UP:
Return to the clinic if symptoms persist or worsen.
